Purpose
Use this scorecard during each trainee’s daily one-to-one precepting session to evaluate execution, SOP adherence, accuracy, efficiency, communication, time tracking, and readiness to advance.
Scoring
Rate each core area from 1 to 5. Record whether the task was complete, the SOP followed, output accurate, timing reasonable, updates proactive, and Hubstaff tracking correct. Record coaching notes and the next-session focus.
Progression Decision
Assess whether the trainee can advance, needs monitoring, requires additional coaching, should have precepting extended, or is not ready. A total of 24–30 is strong, 18–23 needs refinement, and a score below 18 requires an extension.
Critical Flags
Flag incomplete work marked complete, ignored SOPs, missing communication, or use of the wrong system. Any score of 1 or critical failure requires additional review. The SPM should spot-check completed evaluations.
